This form will notify us of your interest in receiving regular literacy intervention with one of our Literacy Specialists. We will call to schedule as soon as you have been placed.

  1. Learn more about our Literacy Intervention Model

  2. Receive a dyslexia evaluation through the INW Dyslexia Alliance

  3. Frequently Asked Questions

To receive intervention services from the Alliance, one of the following is required:

  • A Specific Learning Disability (SLD) diagnosis from school related to reading

  • An official dyslexia diagnosis

  • A WIST evaluation from Cedar Literacy Center

  • A K-2 screener letter from your school stating your child has been flagged for literacy difficulties or indicators of dyslexia (please call your school if you haven’t received a letter and believe you should have).

Please review before moving forward:

  • We suggest a donation of $55-$65 per session to cover operating costs and contribute to the Client Assistance Fund.

  • $50 covers the cost of running each session. This is a tax-deductible donation (other providers may charge cash-paying clients $100+ for a similar service).

  • Any amount pledged over $50 goes into our Client Assistance Fund to help other families in our community who are seeking similar services.

  • Donations for each session will only be accepted via credit or debit card.

  • We encourage you to review our recommended provider page to determine whether the Alliance is the best fit for you based on your insurance coverage, timeframe, and financial needs.

We’re always accepting donations…
